JSFG Souvenir Booklet Family Announcement

At the 2025 JSFG Business Meeting, the family voted to establish a new fee structure for placing family announcements in the JSFG Souvenir Booklet. Family announcements have traditionally been included at no cost. Rising printing expenses have made this difficult to sustain, so the following fee structure has been created to help cover production costs:

  $10.00 - Patron/Name Only
  $25.00 - Quarter Page
  $50.00 - Half Page
$100.00 - Full Page

PAYMENT OPTIONS

•  Register online and click the size for your family announcement. Payment will be included in the grand total. 
• Pay by check or money order. Mailing instructions are on the family announcement form.

ANNOUNCEMENTS, PHOTOS

SUBMISSION
Announcements must be submitted by July 15, 2026 in order to guarantee placement in the JSFG Souvenir Booklet. Announcements submitted after July 15, 2026 cannot be guaranteed placement in the souvenir booklet.

1. Make copies of form: one for your records and one to send if mailing payment.

2. Email form and photos to both of the following:

jsfgprogram@gmail.com and jackprogram2025@gmail.com 

3. If paying by mail, send a copy of form along with check/money order to address on the form.

4. Retain a copy of announcement and payment for your records.
